Afterwards, he pursued world titles at junior welterweight but fell short in championship bouts. In 1991, he moved up to the junior middleweight division. At this division, he captured the USBA title with a 12-round decision over Ron Amundsen. Additionally, he achieved a historic milestone by stopping WBA champion Gilbert Delé in the 12th round. With that victory, Vinny Pazienza became only the second boxer in history to win world championships at both lightweight and junior middleweight.

His momentum was abruptly halted by a devastating car accident that fractured his neck and forced him to relinquish his title. Defying medical expectations, he trained throughout his recovery and returned to the ring just 13 months later. He defeated Luis Santana over 10 rounds in one of boxing’s most remarkable comebacks.

Following his return, he continued to add to his résumé with stoppage wins over Brett Lally and former world champion Lloyd Honeyghan.

In 1993, he captured the vacant IBO middleweight title with an 11th-round knockout of Dan Sherry. He then secured back-to-back unanimous decision victories over legendary former champion Roberto Durán, winning the IBC super middleweight title twice. 

In 1996, he handed Dana Rosenblatt the only loss of his career, a fourth-round knockout, to claim the WBU super middleweight championship. He made a final title attempt in 2002 against Eric Lucas and concluded his career in 2004 with a decision win over Tocker Pudwill.
